School Physical Exams
Pennsylvania School Health Law requires all students to have a physical exam in K/1st, 6th, and 11th grade. We encourage this exam to be completed by your private health care provider and it can be obtained up to 12 months prior to the start of the school year.
If your provate health care provider will be performing the exam, please download the Physical Examiniation Form. The physical exam form should be completed and returned to the school nurse. The first page should be filled out by the parent/guardian and the remaining pages by the provider performing the exam. If you are not able to have your private health care provider perform the exam, your child can be scheduled to be seen by the school physician, Dr. Cordas and his associates. If you plan to have the school physician perform your child’s physical, please fill out the School Physical Authorization form and the first page of the Physical form found in the Health Forms section on the left and return both to the school nurse.
