Meal Balance Request
If your student has withdrawn from the district and has a positive food service account balance of $5 or more please complete the Meal Account Balance Request form below. However if your student owes money on their food service account please send in payment immediately.
Please note that parents of graduating seniors will be mailed a food service account balance form at the end of each school year. A copy of this form is also available below.
As of June 30th of the current school year, if a refund request for your student isn’t received all funds will be donated to assist STSD students.
