School Physical Exams

Pennsylvania School Health Law requires all students to have a physical exam in K/1st, 6th, and 11th grade. We encourage this exam to be completed by your private health care provider and it can be obtained up to 12 months prior to the start of the school year.

If your private health care provider will be performing the exam, please download the physical examination form. The physical exam form should be completed and returned to the school nurse. The first page should be filled out by the parent/guardian and the remaining pages by the provider performing the exam. If you are not able to have your private health care provider perform the exam, your child can be scheduled to be seen by the school physician, Dr. Collina and his associates. If you plan to have the school physician perform your child’s physical, please fill out the School Physical Authorization form and the first page of the Physical form found in the Health Forms section on the left and return both to the school nurse.

School Dental Exams

Pennsylvania School Health Law requires all students to have a dental exam in K/1st, 3rd, and 7th grade. This exam may be done by your private dental provider, or by the school dentist, and can be obtained up to 12 months prior to the start of the school year.

If your private dental provider will be performing the exam, please download the dental examination form found in the forms section to the left.  The private dental exam form should be completed and returned to the school nurse. If you are not able to have your private dental provider perform the exam, your child can be scheduled to be seen by the school dentist, Dr. McNeal. If you plan to have the school dentist perform your child’s exam, you will need to complete the School Dental Authorization form which can be found in the forms section to the left. 

CHIP is short for the Children's Health Insurance Program, Pennsylvania's program to provide health insurance to uninsured children and teens who are not eligible for or enrolled in Medical Assistance. 

There are a lot of reasons kids might not have health insurance — maybe their parents lost a job, don't have health insurance at work, or maybe other options just cost too much. Whatever the reason, CHIP may be able to help.

Nine out of 10 CHIP parents report satisfaction with their child's health plan, and 96 percent received an appointment for checkups and vaccinations as soon as they wanted.

A mobile dentist is coming to STSD the week of October 18^th! 

Parents can sign their kid up at www.myschooldentist.com or click "Smile Program Permission Form" to complete and print out the form.

The mobile dentist will be at the following STSD buildings: 

• Sara Lindemuth: October 23^rd to 27^th
• Thomas Holtzman: October 30^th to November 3^rd
• Middle School: November 6^th, November 7^th, and November 9^th
