Health Requirements

Pennsylvania School Health Law requires that students are up to date on all school-required immunizations (or a letter of exemption) as well as a physical and dental exam in specific grades. We ask that parents/guardians provide:

• Updated immunization records in grades K, 7, and 11 grade.
• A physical exam in K/1st, 6th, and 11th grade
• A dental exam in K/1^st, 3rd , and 7th grade

Pennsylvania School Health Law requires that school nurses perform the screenings listed below. These screenings are typically performed throughout the first half of the school year. Parents/guardians will be notified if any of the screenings result in the need for further follow-up.

• Growth screening (height and weight) in all grades
• Vision screening in all grades
• Hearing screening in K-3rd, 7th, and 11th grade.
• Scoliosis screening in 6th and 7th grade

Transportation

Student bus assignments for the 2025-26 school year are now available in the My Ride K-12 mobile app. Please review the bus stop location(s) for your student(s) at your earliest convenience. If a change is needed, please complete this Transportation Form and submit it ASAP to Transportation@hannasd.org.  For more information visit the Transportation page on our website.
